Checklist: Is My Child Too Sick For School?

Immunization Information

At this time, New York State has not issued extensions for required school immunizations or physicals for the upcoming school year.

TO AVOID EXCLUSION, PLEASE MAKE ARRANGEMENTS AS SOON AS POSSIBLE, as there is only a 14-day grace period allowed once school is in session and HEALTH OFFICES WILL BE BUSY!

There are a few ways to meet this challenge:
  • Call your child's primary healthcare provider to schedule an appointment.
  • Call a local public health office to schedule an appointment: Chenango County Public Health, 607-337-1660; Madison County Public Health, 315-366-2361.
  • Call our School Based Health Center for an appointment: 1-844-255-7242.

School Health Screenings

Please note the following Health Screening Requirements for public schools from the New York State Education Department:
  • Health examinations (physicals) are required for new entrants, Pre-K or Kindergarten, and thereafter in grades 1, 3, 5, 6, 7, 9, and 11. This exam may be performed by your child’s primary healthcare provider or at our School Based Health Center (SBHC). As in the past, a “health history” will be sent to parents/guardians when the physical is due. A copy of the exam must be on file in the student’s medical chart in the Health Office. As in the past, any student participating in a school sport is required to have a physical each year.
  • A dental certificate also will be sent with the health history for new entrants, Pre-K or Kindergarten, and grades 1, 3, 5, 7, 9, and 11. A dental exam is recommended but not required by the NYSED.
  • Hearing screenings will be done by the school nurse in the Health Office. These will be done for all new entrants, Pre-K or Kindergarten, and grades 1, 3, 5, 7, and 11.
  • Vision screenings, to include near vision and distance acuity, will be done for new entrants, Pre-K or Kindergarten, and grades 1, 3, 5, 7, 9, and 11. Color perception will be done on all new entering students.
  • Scoliosis screenings will be done for all female students in grades 5 and 7, and for all male students in grade 9.
All screening results will be documented in the students’ medical chart. If it is determined that your child is in need of a professional examination for any of the above, a written referral will be sent to you. Hearing and vision screenings may be done at any time deemed necessary. Please call the Health Office with any questions/concerns.
